Our Extended Day Program consists of a Morning Care and an After Care. Please complete the form below to register your child for one or both of the programs. Due to COVID-19, registration needs to be completed prior to the use of the program so that St. Ambrose can staff accordingly.

- The following is a list of Policies and Procedures for the Extended Day Program at St. Ambrose. Parents and Guardians interested in sending their children to the program must indicate they they agree to the terms and conditions of the program.
  
  All health and safety guidelines will be followed according to NYS and the Deptartment of Health. These guidelines include:
  
  Students and Staff need to wear face coverings.
  
  Students and Staff will follow proper social distancing procedures.
  
  Students will be given frequent mask breaks.
  
  Cleaning/Disinfecting of tables will be done frequently by staff.
  
  Cleaning and Disinfecting will be done frequently.
  
  The Extended Day Program will be held in the gym. Drop-off/Pick-up of students should follow the Pre-K route (Parents, wearing a face covering, walk in the main entrance).
  
  Fees:
  Morning Care - FREE
  After Care - $8 per day (pick-up before 4pm)
  $16 per day (pick up 4-5:00pm)
  Late Fees will be applied for students picked up after 5:00pm
  Payments for the after-school program are made through your FACTS account. If an account is over one week old and no attempt has been made towards payment, your child will not be allowed into the program until payment is made. You may also pay in school.
  
  Parents are required to provide the program with a list of names and telephone numbers of any adult who will be picking up their child. If someone else needs to pick up your child, please notify the school with a written note. Only in case of emergency can this information be given verbally. Also, please contact us if you will be delayed in picking up your child. The After-School phone number is 518-313-9235
